Essential Vitality 4 Life understands that life happens and that there are several things that may occur that would make it necessary to cancel or reschedule an appointment. However, for Essential Vitality 4 Life to provide excellent customer service and a wonderful experience for all my clients and be fair and accommodating, I have implemented the following policies below.
Booking Your Appointment
You may book your appointment by scheduling at the end of a session, texting or using my online booking calendar. You will then receive a confirmation email with a link to fill out the New Client Information Form before your first appointment. Thank you for filling this out ahead of your appointment.
A credit card on file is now required to book your appointment.
Late Arrival
Please call or text if you are running 15 minutes or more late for your appointment. Depending on subsequent appointments scheduled after yours, we may not be able to provide you with your full scheduled appointment time. Regardless of appointment length, you will be charged the full amount of the original scheduled appointment.
Cancelation and No-Show
I ask that you please reschedule or cancel at least 1 day before the beginning of your appointment, or you may be charged a cancellation fee.
I understand that unanticipated events happen occasionally in everyone’s life. The following policies are honored:
SICKNESS If you need to reschedule because you are not feeling well, I understand and request for you to please contact me as soon as possible to avoid potential missed appointment charges.
OTHER CANCELLATION 24-hour advance notice is REQUIREDwhen cancelling an appointment. This allows the opportunity for someone else to schedule an appointment. If you are unable to give 24 hours advance notice you will be charged the full amount of your appointment. This will be charged to the card on file or must be paid prior to your next scheduled appointment.
NO-SHOW Anyone who either forgets or consciously chooses to forgo their appointment for whatever reason will be considered a “no-show.” They will be charged the full amount for their missed appointment which will be charged to the card on file.
LATE ARRIVAL If you arrive late, your session may be shortened in order to accommodate others whose appointments follow yours. Regardless of the length of the treatment actually given, you will be responsible for the full price of the treatment scheduled. Out of respect and consideration to your therapist and other customers, please plan accordingly and be on time.
Payment
All payments are due in full at the completion of each visit. I accept: Cash, Debit or Credit cards, and HSA cards.
Essential Vitality 4 Life does not accept insurance.
Minor Children
I do offer our massage services to children with parental consent and/or supervision.
Cell Phones
I ask that you please have your phone turned off or on silent upon entering your massage session. This will help you to have your well-deserved relaxation time.
Please read the following statements:
I am aware that the therapist will use draping methods: I will always be covered with a sheet, uncovering only the body part being massaged at the time, while keeping the rest of the body covered during the session.
I understand this is a professional therapeutic massage for the purposes of supporting my health and wellness goals, any occurrence of inappropriate behavior, sexual comments, or sexual advances are never acceptable, and the massage will be terminated immediately.
In this office, massage includes your head, neck, arms, abdomen, legs, back and gluteal (if having low back tightness). Massage of the genitals are NOT performed.
All clients are required to agree to the following Release and Liability Waiver which is effective for all visits.
I understand that Essential Vitality 4 Life Coaching programs, Essential Life Technique Lymphatic Drainage Massage, Swedish Massage and TMJ Release are for improving my overall wellbeing and health. Because massage should not be performed under certain medical conditions, I affirm that I have stated all my known medical conditions and answered all questions honestly. I understand that massage techniques I receive are provided for the basic purpose of lymphatic drainage, relaxation, and relief of muscular tension (depending on type of massage chosen). If I experience pain or discomfort during the session, I will immediately inform the therapist so that the pressure and/or strokes may be adjusted to my level of comfort.
I also understand the therapist does not diagnose or prescribe for medical illness, disease, or any other physical or mental disorder. The therapist also does not perform spinal adjustments/manipulations. I agree to keep the therapist updated as to any changes in my medical profile and understand there shall be no liability on the therapist’s part should I fail to do so. I understand, treatment/s and or programs will not replace conventional medical diagnosis or treatment.
I release Essential Vitality 4 Life, its owner and practitioner from all legal liability during my participation in any and all treatment/s, programs, coaching services, and essential oil use during massage. I assume sole responsibility for my own health and for the results of any sessions provided by Essential Vitality 4 Life. I also understand that coaching programs and massage services are not a substitute for medical examinations and/or diagnosis, and it is recommended that a physician be seen for any medical conditions/ailments I may have. With this in mind, I agree to have this therapy done and hereby release in full and forever discharge Essential Vitality 4 Life Therapist/Owner, Guests, Agents and any and all parties from any and all liability, damages, claims, demands and /or causes of action relating to or deriving from any injury to me during or arising out of the use of the facilities or participation in any therapies including all risk connected therewith, whether seen or unforeseen. I have read and understand the above policies. I have had a chance to ask questions and by signing below, I agree to abide by these policies.
